A senior manager earning $180,000 plus a $40,000 bonus ($220,000 total): Social Security withholding stops at $176,100. Additional Medicare Tax (0.9%) applies to income above $200,000 — on $20,000 of income = $180 in extra tax. The bonus of $40,000 is withheld at 22% flat ($8,800 withheld) regardless of actual effective rate. At a 28% effective rate, the bonus withholding is insufficient by $2,400.
Managing Bonus Withholding Through W-4
If your bonuses are regularly under-withheld at the 22% flat rate (and your effective rate is higher), use Step 4c to add extra withholding per regular paycheck throughout the year. This pre-pays tax on the bonus income so the flat-rate withholding on the bonus itself is sufficient. Example: $40,000 bonus at 28% effective rate under-withheld by $2,400; add $92/paycheck (26 periods) in Step 4c to compensate.
Employers are required to withhold the Additional Medicare Tax (0.9%) from wages above $200,000 for single filers. This starts automatically when wages cross that threshold — no W-4 action needed. For MFJ couples: the threshold is $250,000 combined, but each employer withholds based on individual wages, not combined — a couple may under-withhold the 0.9% tax and owe at filing.
